Prevention of Medication Errors (Spain)
The members of Spanish Group for the Prevention of Medication Errors (GEPEM) of the Spanish Society of Hospital Pharmacy (SEFH) are hospital pharmacists interested in working to improve safety in the medication use system.
In 2004, the group was created by the SEFH, which acts as motivator and sponsor for the group’s activities. Such a group came into being as a result of a conviction that it was urgent and unavoidable to confront the problem and to join together the efforts of hospital pharmacists as a group.
The main activities that have been carried out are:
- Developing training activities for hospital pharmacists designed to improve safe use of medications;
- Publishing a document that defines the professional role of the hospital pharmacist in the prevention of medication errors; and
- Carrying out research in the field of medication safety.
The group is finishing a survey on the situation of hospital pharmacy with regard to activities for managing medication risks, and is designing a national study to determine the prevalence of medication errors among hospitalized patients.
