GP.51. Reporting system on sentinel events for hospitals

National Reporting System Sentinel Events (Slovenia)

On the national level a reporting system on sentinel events for hospitals was launched in 2002.

The system is based on JCAHO sentinel events format and is both anonymus and confidential. An event
is analyzed by hospital (Root Cause Analysis) and feedback is than provided by the Ministry of Health.

Only the name of the hospital in question is known to the ministry. Results are anonymously available to public.

This project has raised some awareness among leadership and professionals about safety, but only few hospitals have taken it seriously, because a culture of fear is still prevailing in Slovenia.
