Notification of Adverse Events and Incidents Registry (Portugal)


On a local level a good practice example is represented by two hospitals that have created local systems of notification of adverse events and incident registry.

These two hospitals have initiated accreditation processes in the last 4/5 years and these systems emerged as one result of that successful quality improvement project. They have started with risk management tools and activities and moved to incident registry, study and action implementation.
