GP.47. Blame free reporting in neonatology (Isala clinics, Zwolle)

Blame Free Reporting (Netherlands)

In the Isala clinics in Zwolle the neonatologist Harry Molendijk (current chair of the Dutch platform on patient safety) introduced a system of blame free reporting into his department. As a result there was a substantial increase in the number of reported incidents.

This system is now seen as a good practice example for the rest of the Netherlands.
