GP.43 Safer prescribing by redesign of drug charts within 36 acute hospitals in the Tuscany region

Medication Prescription (Italy)
A desire to reduce prescribing errors and improve patient safety led a team at the Tuscany CRM centre to design, using ergonomic methods, an Integrated Therapeutic Chart, which was then piloted throughout the Region to test compliance, usability and effect on medication errors across a range of specialities. The outcomes were positive.
