Risk Identification (Italy)


A 2nd paper discusses the setting up of the information system needed to service the activities of the CRM centre.

The system basically relies on two main sources of information about clinical risks and accidents inside hospitals:

    - patient claims (coming from the different healthcare local units are collected in a network-based integrated regional database), and

    - clinicians voluntary reports (a system of incident reporting collects the voluntary reports by the clinical operators concerning unsafe actions, near miss or incident without harm for the patient).


In the next months, the system will be tested by the healthcare local agencies, the clinical risk managers and the Clinical Risk management and Patient safety centre (CRM centre). The Tuscany model is an innovative system in Italy as it integrates the risk monitoring system to the risk analysis and management system.
