Incident Reporting (Italy)
An incident recording system was tested over 25 wards in eight hospitals within the Veneto region.
In total 360 events occurring over the nine months of the pilot project indicated errors in drug administration as the leading source of error. The aim was subsequently to implement the scheme throughout the region.
