GP.25. The Securimed Project: assessment of the medication system in 19 hospitals in the Aquitaine region

Assessment of a Medication System (France)


Medication safety is a key theme of the workplan of CCECQA (Comite de Coordination de l'Evaluation Clinique et de la Qualité en Aquitaine) - the Evaluation and Quality Coordination Committee. The CCECQA has developed an assessment method of the medication circuit system based on a risk-assessment visit.

The project, entitled SECURIMED "SECURIsation du Circuit du MEDicament", conducted from June 2003 to 2005, aims to study the safeguards set up in order to prevent errors during the different stages of the medication system (MS) and to help the hospitals to define improvement programmes:

  • 21 pharmacy departments and 38 healthcare units in 19 hospitals located in
    the Aquitaine region (southwest of France) participated in the study.

From June 2003 to April 2004, risk-assessment visits were conducted in two care units and the pharmacy departments of each volunteer hospital. The visit included:

    * a presentation of the unit carried out by the supervisors,
    * individual interviews with the supervisors and the healthcare workers,
    * an observation of each stage of the MS,
    * a study on the difference between the prescribed medicine and the administered medicine,
    * the analysis of an adverse event scenario,
    * an analysis of the available documents regarding the MS, and
    * a feedback meeting.
