Table of Content WP2 'Best Practice Compendium'



This compendium is a companion document to the final report on the mapping exercise for the project. In addition there is an accompanying document (Annex 1), which has a number of the larger files printed out, in which case they are briefly summarised below with appropriate cross-references. Annex 2 is an additional supplement, with two items in PowerPoint format.

Important note: the basis for inclusion in the compendium was nomination by other experts. Therefore there is no formal evidence base for these examples being good practice, nor validation of content other than the expert opinion of the nominating person.

Preface


A note on the proposed classification of good practice examples (to be used as the basis for entry into the compendium database):

The examples of good practice are grouped within three domains:

    1. Reporting and learning systems
    2 .Education and training about patient safety
    3. Quality improvement projects on patient safety

Inevitably there may be arbitrariness about allocation to a category, but the database does offer two types of search facility, by category or via a ‘Google’ type search for word or phrase.

The domain ‘Reporting and learning systems’ has two main sub-categories (divided into national/local projects):

    a) reporting systems (with a sub-category for reporting sentinel events only); and
    b) risk management projects.

The domain ‘Training and education’ includes sub-categories, of:

    a) education on patient safety (national and local);
    b) campaigns (national and local, specified by topic); and
    c) national promotion of guidelines/indicators in patient safety (by topic).

The domain ‘Quality improvement projects’ has three sub-categories:

    a) medication safety projects (national or local);
    b) culture of safety projects (national or local); and
    c) patient safety improvement projects (national or local, specified by topic)
